Long Term Care

Most of us will require some type of care assistance as we grow older. We may be used to managing everything ourselves or with our spouse, or receiving support from family members for minor help around the home. But in time our circumstances change, getting around and taking care of ourselves can become more of a challenge. Having a little support with day to day tasks can make all the difference to maintaining independence and quality of life.

 

It’s natural to want to stay at home as you grow older. The familiar can be comforting and your home is likely filled with fond memories and your neighbourhood with familiar people. However, taking a step back to look at the big picture can help you decide whether staying at home for the long term truly is the right step for you. Earlier planning and examining which home care services are best for you can make it easier to make the choice that’s right for both you and your family. 

 

Everyone needs variety, depending on factors such as how much support you already have, your general health and mobility. Our trained care planning team can support in assessing your needs and preferences for support and will continue to support you and develop the plan as your needs change.

 

You may only want assistance once a week or you may need several calls a day, every day.

Personal care may include; help with washing, bathing or showering, oral care, hair and nail care, assistance with medication, continence care, getting dressed/undressed or help with meal preparation and meal support.

 

Our trained and compassionate care staff visit you at pre-arranged times and follow your care plan to ensure all your needs are met. However, we appreciate everyone’s needs can change and unexpected events happen now and then, so our staff are flexible in how they support you during each care visit.

Example of the services that we can support you with include:

 

Household support: Keeping your home running smoothly can take a lot of work. If you’re beginning to find it hard to keep up, we can support you with laundry, shopping, gardening, housekeeping, and handyman services. We can also help you to comfortably stay on top of bills and appointments.

 

Personal care: We can help with the activities of daily living, such as dressing, bathing, or meal preparation which is described as personal care. This can be arranged to take place in one short care call per day to around-the-clock live-in care. We can also provide assistance with medication collection and management.

 

Community Access: Our care and support staff can enable you to access the community to run errands, do your shopping, attend appointments and go on day trips and outings with you.


 

Home care (domiciliary care) can be arranged on an hourly, daily or weekly basis for a single visit or full time care.


 

Aspects of personal care provided includes the following:

  • Domestic services as part of a care package or independently

  • Live in service

  • 24 hour care

  • Night sitting or sleep over service

  • Getting up or putting to bed service

  • Hospital discharge service

  • Shopping services

  • Laundry services

  • Bathing services

  • Meal preparation and meal support

  • Outings and special events as an escorting service

  • Holiday escorting services (including foreign holidays)

  • Liaison with social workers, district nurses, family and friends.
